New Patient Visit, Naturopathic- In person or Virtual:

Upon scheduling your appointment, you will receive an email confirmation and a link to your personal client portal account. All of your new client intake forms will be available through the portal and can be completed electronically. You are required to complete all paperwork PRIOR to your first visit, as this information will allow for efficient time utilization during your visit. Additionally, please bring / send electronically any recent health records or lab work to your visit.

Your first office visit will be comprehensive; it will cover your current health concerns,
medical history, as well as diet and lifestyle habits. If time allows, a pertinent physical exam (not included in virtual visits) will be performed, as well as an individualized treatment plan focusing on your treatment goals. Please allow 90 minutes for adult New Patient Visit, and 60 minutes for pediatric (17 years old or younger).

Follow up visits will be necessary to allow for assessment of therapeutic progress, discuss laboratory results (if ordered), and provide necessary modifications to your treatment plan.

New Patient Visit, Chiropractic:

Chiropractic first office visit will include a focused intake, physical examination, as well as a manual therapy session. Please allow 60 minutes for Chiropractic new patient visits.

Acupuncture:

Includes a focused intake and an individualized therapy session. Please allow 45 minutes for acupuncture visits.

Follow Up Visit- In Person, Virtual, or Phone Consultation:

A return office visit may be scheduled for 30 or 60 minutes (you will be charged according to the time used). A follow up visit may be based upon your previous visit OR to address a new concern.

Follow Up Visit- Chiropractic:

Please expect follow up visits to last approximately 15-25 minutes.

Dr. Liebich is in network with Blue Cross Blue Shield IL.

Payment Policy:

All payments are due in full at the time of service. Cash, checks, and major credit cards are accepted methods of payment.

Past-due amounts:

Past due accounts that show to have zero activity for more than 90 days will be turned over to a collection agency.

Cancellation Policy:

Cancellations or appointment rescheduling must be completed 48 hours prior to appointment. If this is not followed, a 70% charge of the scheduled visit will incur.
